Perseverance Rehab & Performance ("we," "us," or "our") is committed to protecting your privacy. This policy explains what information we collect when you visit our website or contact us, how we use it, and your rights regarding that information.

This website is informational. We do not provide medical advice, diagnose conditions, or maintain patient health records through this site. Clinical records are handled separately under applicable healthcare privacy laws.

We collect information in two ways:

• Contact form submissions — When you fill out the contact form, we collect your name, email address, phone number, age, primary instrument(s), current situation, any upcoming tour dates or travel you share, your timezone, your occupation, and any details you provide about your goals, activities, pain, or medical history. You choose what to share.
• Automatic data — Our hosting provider (Firebase / Google Cloud) may log standard server data such as IP addresses and browser type as part of normal infrastructure operation. We do not review or use these logs for marketing purposes.

How We Use Your Information

Information submitted through the contact form is used solely to:

• Respond to your inquiry
• Schedule an initial consultation
• Follow up on your request if we do not hear back

We do not use your information for marketing, advertising, or any automated decision-making. We do not sell, rent, or share your personal information with third parties for their own use.

Data Retention

Contact form submissions are delivered to us by email. We retain that email correspondence only as long as necessary to complete your inquiry or begin a client relationship. If you do not become a client, we will not retain your information beyond the scope of our initial communication.

Third-Party Services

We use a small number of third-party services to operate this website:

• Cloudflare Turnstile — Used on our contact form to detect automated (bot) submissions. Turnstile may set a strictly functional cookie. No personal data is shared with Cloudflare beyond what is necessary for bot verification. See Cloudflare's privacy policy.
• Firebase (Google Cloud) — Our website and contact form backend are hosted on Firebase. Google may process request data as part of hosting infrastructure. See Google's privacy policy.
• Zoho Mail — Contact form submissions are delivered to us via Zoho's email service. See Zoho's privacy policy.

Cookies

We do not use tracking cookies, analytics cookies, or advertising cookies. The only cookie that may be set is a strictly functional one by Cloudflare Turnstile for spam prevention on our contact form. No cookie consent banner is shown because no non-essential cookies are used.

Your Rights

Depending on where you live, you may have rights regarding your personal information — including the right to access, correct, or request deletion of data we hold about you. To exercise any of these rights, contact us directly at the email below. We will respond within a reasonable timeframe.

Children's Privacy

This website is not directed at children under the age of 13. We do not knowingly collect personal information from children. If you believe a child has submitted information through our site, please contact us and we will delete it promptly.

Changes to This Policy
